Arc Regen Health was built to close the gap between leaving the hospital and getting settled at home.
Our approach
Too many patients leave the hospital and then lose contact with their care team. Missed follow-ups, medication mix-ups, and wounds that stop healing send people back to the hospital.
We send a provider to the patient. Our physicians and nurse practitioners see patients at home and by telehealth, coordinate with home health and hospice teams, and keep referring providers informed.
